JPS is making the COVID-19 vaccine available to JPS patients, JPS vendors, and employee family members living in the employee's household who meet at least one of the following criteria:
- 65 years of age and older
- 18 years of age and older with at least one chronic medical condition that puts them at risk for severe illness from the virus that causes COVID-19
You must have an appointment to receive the vaccine. Appointments can be scheduled two ways:
- through JPS MyChart
- over the phone at 817-702-1100
Vaccines are available at the following JPS locations:
- Southeast Medical Home
- Northeast Medical Home
- Stop Six-Walter B. Barbour Health Center
- Diamond Hill Health Center
- Polytechnic Health Center
- Family Health Center
- Gertrude Tarpley-Watauga Health Center
- Viola Pitts-Como Health Center
- Magnolia Health Center
- South Campus Health Center
- True Worth Medical Home
